About C-Trilogy
Our Story
In 2015, nurse practitioner Dr. Tracy Hicks, NP founded C-Trilogy, a for-profit practice focused on providing high-quality mental health care in East Texas. As the clinic grew, Dr. Hicks witnessed the number of people were falling through the cracks, especially during the COVID-19 pandemic, unable to access services due to cost, transportation or stigma.
That’s why in 2020, she launched C-Trilogy Outreach, the nonprofit arm of the clinic, and in 2022 C-Trilogy became a nationally recognized Certified Community Behavioral Health Clinic (CCBHC). Our mission? To make care more equitable, more integrated, and more accessible for every person regardless of background, income or insurance status.

At the heart of C-Trilogy Outreach is Project Holistic Health & Equity, our bold initiative to remove the barriers that keep people from getting the care they deserve.
We know that health care isn’t just about scheduling an appointment. It’s about trust, convenience, affordability, and feeling truly safe and seen. That’s why we offer integrated, whole-person care designed to meet your physical, mental, and emotional needs — all in one place.
Project Holistic Health & Equity
By combining primary care, mental health care and substance use services under one roof, we help:
Reduce stigma by treating emotional and physical health together
Catch issues early with collaborative teams who see the full picture
Make care easier by offering everything in one visit
Close gaps in access, especially for historically underserved communities
Build long-term trust, through consistent, compassionate relationships
Whether you're managing mental health conditions, living with a chronic condition, recovering from substance use, or just looking for support, we’re here for you. Our team works together to make sure your care is connected, respectful and tailored for your needs.
